右室收缩时间间期与肺动脉压力(附主肺动脉压力曲线分析)

摘要 对43例行右心导管病员的主肺动脉压力曲线以及由其导出的右室收缩时间间期指标与肺动脉压力的相关关系进行了分析。结果表明:①右室射血前期,右室射血前期与右室射血期的比值与心率不相关,但右室射血期与心率有一定程度的相关关系(r=-0.685,p<0.01);②右室射血前期,右室射血期与肺动脉压相关均较差,但右室射血前期与射血期的比值与肺动脉压有较好的相关关系,且不需进行心率校正。肺动脉压的多元回析分析表明,采用多个因素估测肺动脉压可提高准确性。这些结果为无创性方法估测肺动脉压提供了可靠的依据。 Forty—three main pulmonary artery pressure tracings obtained from patients by right cardiac catheterization were analysed. The correlation between parameters of right ventricular systolic time intervals and pulmonary artery pressure (PAP) was studied. We concluded: ① Right preejection period (RPEP) and the ratio of RPEP to right ventricular ejection time (RVET), i.e., RPEP/RVET did not correlate with heart rate (HR), but RVET correlated with HR in some degree (r=-0.685, P<0.01). ② RPEP and RVET correlated poorly with PAP, while RPEP/RVET correlated relatively well with PAP and needn't be corrected for HR. The multiple variable analysis suggests that reliability of predicting PAP may be improved. These results provide reliable evidence for noninvasive prediction of PAP with RVSTI.
