840例食管癌放射治疗临床分析

CLINICAL ANALYSIS OF 840 PATIENTS WITH ESOPHAGEAL CANCER TREATED BY RADIOTHERAPY

我院放疗科自1976年1月15日至1981年12月31日住院放射治疗肿瘤量在4000cGy以上的食管癌840例,现总结分析如下:

840 patients (male/female 591/249) with esophageal cancer as treated by radiotherapy (40 Gy) , from 1975 to 1981, in our department were analysed. Lesion were located in the upper segments in 193 cases (23%) , mid-segments in 593 cases (70.6%) and lower segments in 54 cases (6.4%) .591 cases (70.4%) were confirmed by pathalogy or cytology. The length or the lesion in 19 cases (2,3%) was shorter than 3 cm, in 456. cases (52.3%) between 3-5 cm, in 329 cases (39.2%) between 5.1-8 cm and in 36 cases (4.3%) longer than 8 cm. All patients were treated with 60Co. The 3 or 4 field techique was used in 71 cases (8.5%) , 286 cases (31.9%) and 501 cases (59.6%), respectively. Tumor doses were 5000-7000 cGy/5-7 wk.All patients have been followed for over 5 Years, The 1, 3 and 5 Year survival rates were 58.2%, 22.4% and 15.5%, respectively.The radiotherapeutic response was found to be closely related to the age, location length, DT and degree of response of the lesion. Local tumor unqontrol and recurrence were the main causes of failure
