鱼油20碳5烯酸对冠心病血小板功能的影响

摘要 20例冠心病患者口服国产鱼油提取的EPA(20碳5烯酸),服药21天,每天8mg/kg。停药后第1天及第15天血小板粘附试验(Salzman法)从服药前的84.65±3.28%分别减至97.45±7.08%(P<0.001)及45.85±10.34%(P<0.001)。ADP(2UM)诱导的血小板聚集试验从服药前的68.11±11.91%分别减至59.38±9.35%(P<0.025)及61.35±12.74%(P<0.05)。肾上腺素(0.4mg/L)诱导的血小板聚集试验从服药前的71.16±13.02%分别减至62.26±11.41%(P<0.05)及60.24±15.38%(P<0.05)。6-酮PGF_1α水平则从服药前的12.86±2.83qg/ml分别增至18.93±9.11pg/ml(P<0.01)及18.04±7.92pg/ml(P<0.01)。TXB_2水平从服药前的388.25±242.59pg/ml减至220.25154.48pg/ml(P<0.01)及181.5±113.48pg/ml(P<0.01)。停药后第一天血胆固醇水平从服药前的215.34±41.95mg%降至186.83±34.30mg%(P<0.01)结果显示EPA有抑制血小板功能,增加PGI_2形成的作用,是预防和治疗冠心病的一个有希望的药物。 20 cases with ischemic heart disease have been treated with EPA(eicosa- pentaenoic acid), an extraction from fish oil, in dosage of 0.008 gm/kg/day per os for 21 days. On the first day and 15th day after suspension of the drug, it was showed that the platelet retention test measured by Salzman method redu- ced from original 84.65±3.28% to 67.45±7.08% (P<0.001) and 45.85±10.34 %(p<0.001)respectively, while the intensity of the platelet aggregation in- duced by ADP 2μm, reduced from original 68.11±11.91% to 59.38±9.35% (P< 0.025) and 61.35±12.74%(P>0.05), that induced by adrenalin (0.4mg/1), from original 71.16±13.02% to 62.26±11.41%(P<0.05)and 60.24±15.38%(P< 0.05), The 6-oxo-PGF1α level measured by RIA increased from original 12.86 ±2.83Pg/ml to 18.93±9.11 and 18.04±7.92Pg/ml on the first and 15th day after discontinuing the drug, while the TXB_2 level measured by RIA decreased from original 388.25±242.59 to 220.25±154.48(P<0.01)and 181.5±113.48 pg/m (P<0.01). The blood cholesterol was lowered 13.2%(P<0.01).There were no significant side effects. The results showed that EPA might be a promising drug in the prevention and treatment of athero-sclerosis and IHD as it inhibited platelet functions and increased the production of PGI_2.
