骨痛膜对日本大耳白兔膝关节骨性关节炎骨内高压和血液流变学作用的实验研究

Experimental Study of Gutong Film on Intraosseous Hypertension and Hemorrheologic Change in Knee Osteoarthritis of Rabbits

目的:观察骨痛膜对日本大耳白兔膝关节骨性关节炎骨内高压和血液流变学的作用,为临床治疗膝关节骨性关节炎(OA)提供实验依据。方法:将日本大耳白兔随机分为正常组、造模组、治疗组(骨痛膜组)、对照组(通络祛痛膏组)、基质膏组,除正常组外,均行静脉结扎建立兔右下肢骨内高压模型。造模5周后分别以骨痛膜、通络祛痛膏、基质膏外用,治疗3周测量兔双下肢胫骨上端的骨内压,然后取造模侧胫骨上端骨髓血测量血液流变学变化,骨内压测量结果采用t检验,骨髓血血液流变学检测结果采用q检验。结果:造模组骨内压较正常组明显增高(P<0.01),骨髓血血液流变学状态较正常组有明显改变(P<0.01),骨痛膜组与通络祛痛膏组均有降低骨内压作用(P<0.01),血液流变学状态更趋于正常(P<0.01),两组的骨内压和血液流变学状态指标与正常组比较差异无统计学意义(P>0.05)。结论:骨痛膜外贴能有效地降低日本大耳白兔膝关节骨性关节炎骨内高压,明显改善骨髓血血液流变学状态,并可使其恢复至接近正常水平,从而起到延缓关节软骨的退变,有利于软骨修复的作用。

Objective：To observe the effect of Gutong Film on and hemorrheologic change in rabbits with knee osteoarthritis（OA） so as to provide experimental basis for the clinical therapy of knee OA.Methods：Japanese big eared white rabbits were randomly divided into normal group,model group,Gutong Film（GTF） group,Tongluo Qutong Plaster（TQP） group and matrix plaster（MP） group.The animal model of right knee intraosseous hypertension was made by ligation of the right femoral vein and inferior gluteal vein.After 5 weeks,GTF,TQP and MP were respectively used for 3 weeks.Then intraosseous pressure of upper end of tibia in both lower limbs was measured.Hemorrheology was also measured by drawing marrow blood of upper end of tibia of the right lower limb.Results： The intraosseous pressure of the model group was obviously higher than that of the normal group（P〈0.01）.Hemorrheology of marrow blood of the model group changed greater than that of normal group（P〈0.01）.Both GTF and TQP effectively lowered the intraosseous hypertension（P〈0.01） and the hemorrheology tended to return to normal level（P〈0.01）.There was no significant differences in the changes of intraosseous hypertension and hemorrheology（P〉0.05）.Conclusion：The external use of GTF can effectively lower intraosseous hypertension caused by knee OA,distinctly improve the hemorrheology of marrow blood and enable it to recover its normality,so that the degeneration of joint gristle is delayed and the gristle is better recovered.
