近视患者应用阶梯渐进衍射型多焦点人工晶状体的初步观察 被引量:7

Preliminary result of apodized diffractive multifocal IOL implantation in cataract patients with myopia

摘要 目的评价阶梯渐进衍射型多焦点人工晶状体(IOL)应用于近视患者的临床效果及安全性。设计前瞻性病例系列。研究对象近视合并白内障患者30例(44眼)。方法对以上患者行白内障超声乳化吸除术,按自愿原则分别植入Acrysof ReSTOR多焦点IOL14例(22眼)或普通单焦点IOL16例(22眼)。比较两组患者术后1周、1个月、3个月远、近视力及焦点深度范围。主要指标裸眼远近视力、矫正远近视力、最佳矫正远视力下近视力及焦点深度范围。结果阶梯渐进衍射型多焦点IOL组患者和单焦点IOL组患者的术后3个月平均裸眼远视力分别为0.84±0.14、0.70±0.23(P=0.018);平均裸眼近视力分别为0.69±0.34、0.32±0.13(P=0.000);矫正远视力下近视力分别为0.69±0.20、0.27±0.12(P=0.000);焦点深度范围分别为(5.54±0.53)D、(3.57±0.93)D(P=0.000)。结论近视患者植入阶梯渐进衍射型多焦点IOL可获得良好的远、近视力,效果满意。 Objective To evaluate the clinical result and safety of phacoemulsification and Acrysof ReSTOR multifocal intraoeular lens (MIOL) implantation in cataract patients with myopia. Design Prospective case series. Participants 30 patients (44 eyes) with cataract and myopia. Methods 30 patients underwent phacoemulsifieation and IOL implantation were divided into two groups: Aerysof ReSTOR group (MIOL) with 14 patients (22 eyes) and monofocal/single intraocular lens (SIOL) with 16 patients (22 eyes). At 1 week, 1 mouth, 3 months after surgery, distant and near visual acuities were observed. Depth of focus was measured 3 months postoperatively. Main Outcome Measures Uncorrected distant visual acuity (UCDVA), uncorrected near visual acuity (UCNVA) , best corrected distant visual acuity (BCDVA), best corrected near visual acuity (BCNVA), distant corrected near visual acuity (DCNVA), depth of focus. Resuits At 3 months postoperatively, UCDVA, UCNVA, DCNVA were 0.84±0.14, 0.69±0.34, 0.69±0.20 respectively, and depth of focus was 5.54±0.53 D in MIOL group. UCDVA, UCNVA, DCNVA were 0.70±0.23, 0.32±0.13, 0.27±0.12 respectively, and depth of focus was 3.57±0.93 D in SIOL group. There were significant differences between two groups in all parameters above(all P〈0.05 ). Conclusion Acrysof ReSTOR (MIOL) implantation in patients with cataract and myopia can provide better visual quality both in distant and near visions and less dependence on spectacles than SIOLs implantation.
