自体冷血停跳液对未成熟心肌三磷酸腺苷酶的影响

The influence of autoblood cardioplegla on ATPase in neonatus

摘要 目的 研究自体冷血停跳液对非紫绀先天性心脏病患儿心肌细胞ATP酶的影响,探讨自体冷血停跳液对未成熟心肌的保护作用。方法体重≤8kg非紫绀先天性心脏病婴幼儿30例,其中室间隔缺损(VSD)2例,VSO伴肺动脉高压(PH)11例,VSD伴房间隔缺损(ASD)伴PH9例,ASD2例。VSD伴卵圆孔未闭(FPO)6例。随机分为自体冷血组、冷血组和晶体停跳液组,每组10例。分别于心脏停跳前、复跳后取右心耳心肌。检测心肌ATP酶的活性。结果手术前、后自体冷血组、冷血组和晶体停跳液组心肌细胞Na+K+-ATP ase[(4.84±1.48)μmol/(mg·h)与(3.76±1.42)μmol/(mg·h)、(4.82±1.30)μmoL/(mg·h)与(2.56±1.32)μmol/(mg·h)、(4.72±1.42)μmol/(mg·h)与(2.12±1.24)μmol/(mg·h)],Ca^2+-AT—Pase[(10.02±1.64)μmo]/(mg·h)与(8.56±1.45)μmol/(mg·h)、(9.87±1.48)μmoL/(mg·h)与(5.87±1.42)μmol/(mg·h)、(10.23±1.56)μmol/(mg·h)与(4.91±1.36)μmol/(mg·h)]和Ca^2+-Mg^2+-ATPase[(24.23±2.01)μmoL/(mS·h)与(19.21±1.93)μmol/(mg·h)、(23.18±1.86)μmol/(mg·h)与(12.32±2.01)μmol/(mg·h)、(24.02±2.32)μmol/(mg·h)与(10.12±1.87)μmol/(mg·h)]活性差异均有统计学意义(p均〈0.05);术前自俸冷血组与冷血组和晶体停跳液组比较差异均无统计学意义(P均〉0.05),术后冷血组和晶体停跳液组与自体冷血组比较差异均有统计学意义(P均〈0.05)。结论自体冷血停跳液对未成熟心肌ATP酶影响最小,对婴幼儿心肌具有良好的保护作用。 Objective To study the influence of autoblood cardioplcgia on ATPase in neonatus myocardium with congenital heart diaease and approach the mechanism of self-blood eardiople^ia in protecting the myocardium in neonatus. Methods There were 30 cases of neonatus with congenital heart disease with body weight less than 8 kg, including 2 cases of ventricular septal defect( VSD), 11 of VSD with severe pulmonary hypertension(PH) ,9 cases of USD with ASD,2 cases of atrial septal defect (ASD) ,6 of VSD and FPO. 30 neonatus were divided into autoblood cardioplegic solution group (group A , n = 10 ), allograft blood cardioplegie solution group (group B, n = 10) and crystalloid cardioplegic solution group( group C, n = 10). The biopsies were taken from fight atrium just before arrested and after heart self-recovery to measure ATPase. Results Comparing with preoperative one, Na+ -K+ -ATPase [(4.84±1.48) μmol/(mg·h) vs (3.76±1.42) μmol/(mg· h),(4.82±1.3)μmol/(mg· h) vs (2.56± 1.32) μmol/(mg· h),(4.72±1.42)μmol/(mg · h) vs (2. 12± 1.24)μmol/(mg · h)] ; Ca^2+ATPase [ ( 10.02±1.64) μmol/( mg · h) vs ( 8.56± 1.45 ) μ mol/( mg · h ), (9.87±1.48 ) μmol/( mg · h) vs ( 5.87 ± 1.42), ( 10.23 ± 1.56) μmol/( mg· h) vs (4.91 ± 1.36) ] μmot/( mg · h) ] and Ca^2+ -Mg^2+ -ATPase[ (24.23 ± 2.01) μmol/(mg · h) vs (19.21±1.93) μ mol/(mg · h),(23.18 ±1.86)μmol/(mg · h) vs (12.32 ±2.01) μmol/( mg· h ) , ( 24.02 ± 2.32 ) μmol/( mg· h ) vs ( 10.12 ± 1.87 ) μmol/( mg · h ) ] in myocardial cell were decreased obviously after operation in group A, B, C ( P 〈 0.05 ). There had no significant difference among the three groups before operation (P 〉0.05 ) . After operation, myocardial cell's Na^+ -K^+ -ATPase, Ca^2+ -ATPase and Ca^2+- Mg^2+ -ATPase in group A were decreased obviously as compared with that in group B and C (P 〈 0.05 ). Condusion There is slight influence of autoblood cardioplegia on ATPase in neonatus with congenital heart disease, which can give a good protection to the myocardium in neonatus.
