摘要
目的观察肠梗阻时,奥曲肽对肺组织有无保护作用.方法SD大鼠40只,随机分成实验组和对照组,每组20只.距屈氏韧带25cm处结扎小肠,实验组立即皮下注射奥曲肽4μg/kg,12h重复1次.对照组给予同体积的生理盐水.24h后,测各扩张肠管的直径,测血液中内毒素和磷脂酶A2的浓度,取肠管和肺组织做病理学检查.结果实验组肠管的直径为64mm±06mm,而对照组肠管的直径为77mm±06mm,两者相差非常显著(P<001).实验组内毒素浓度(kEU/L)029±003,对照组为036±007,两组有非常显著性差异(P<001).实验组PLA2浓度(U)为25±8,对照组为40±15,两组相差非常显著(P<001).而实验组肠和肺组织的病理改变均轻于对照组(P<005).结论在肠梗阻时,奥曲肽对肺组织有保护作用。
AIM To find out wheather octreotide can protect lung from damage of intestinal obstruction. METHODS Forty male Sprague Dawley rats were randomly assigned to octreotide group and control group. The intestine was ligated 25cm from Treitz ligament. Octreotide group received octreotide (4μg/kg) subcutaneously, and repeated 12 hours later. The control group received the same volume of saline. After 24 hours, diameters of all obstructed segment were measured, and blood endotoxin and phospholipase A2 was determined. Samples of obstructed intestine and lung were obtained for pathological examination. RESULTS The diameter of intestine was 6 4mm±0 6mm in octreotide group and 7 7mm±0 6mm in control group, the difference was significant ( P <0 01). The concentration of endotoxin (kEU/L) in octreotide group was 0 29±0 03, while in control group it was 0 36±0 07, the difference being significant ( P <0 01). The concentration of phospholipase A2 in octreotide group was 25U±8U, and 40U±15U in control group, the difference was also significant ( P <0 01). There was more evident pathological changes in obstructed intestine and lung in control group than in octreotide group ( P <0 05). CONCLUSION Octreotide can protect lung from damage in intestinal obstruction by decreasing the phospholipase A2 and endotoxin level.
