正常人脑白质磁共振成像高信号的危险因素和临床意义

Risk factors and clinical significance for white matter highintensities detected by MRI in healthy elderly

检查１５８名正常人的脑磁共振成像（ＭＲＩ），发现４９名在Ｔ２加权像（Ｔ２ＷＩ）上显示白质高信号（ＷＭＨｓ）。分析ＷＭＨｓ与年龄、脑卒中危险因素及眼底动脉硬化间的关系发现，ＷＭＨｓ与年龄有显著相关性（Ｐ＜０．０５）；与脑卒中危险因素有非常显著相关性（Ｐ＜０．０１），且多个危险因素同时存在时其作用有叠加效应；与眼底动脉硬化间亦有显著相关性（Ｐ＜０．０５）。提示ＷＭＨｓ具有与脑卒中相同的危险因素，其出现可能导致老年人的认知功能减退、步态异常及平衡障碍。

MRIs were perfomed on 158 persons(range 46 to 78 years) without neurological symptom so as to find out the risk factors and clinical significance of white matter high intensities(WMHs).49 persons were shown WMHs on T2WIMRI.WMHs were graded using a 4level grading score.The scores of WMHs were significantly related to the age(P<0.05),the risk factors for stroke(P<0.01).In addition,multirisk factors on a person presented an additive effect.It was related to the fundus arteriosclerotic(P<0.05).It was suggested that the risk factors for WMHs may be as same as those for stroke.It was possible that WMHs may be associated with poor cognitive functioning,gait disturbance,and balance dysfunction of neurologically healthy elderly.
