胆总管十二指肠舌样切除吻合术治疗胆管结石

TONGUE SHAPE RESECTION OF CHOLEDODUODENUM WITH CHOLEDOCHODUODEN OSTOMY FOR COpLAN GL-OLITHASIS

逆行胆管炎、官端综合征或再生结石仍然是胆肠吻合术后一个难题。我院采用胆总管十二指肠舌样切除吻合术治疗胆管结石12例，结果表明，该组病人无术后并发症和死亡，两年随访未发现病人出现胆管炎、盲端综合征再生结石。钡餐检查显示，虽然钡剂逆流入胆道但均在6～10min内排出。该术式的主要优点是吻合口大，不发生胆汁和肠内容物滞留且简单安全。本文对该术式的治疗机理、适应证和术中注意事项进行了讨论。

The ascending cholangitis, blind tubule syndrome, or regenerative stones are still challenges after choledocoenterostomy. 12 patients with cholangiolithasis were treated by tongue shape resection of choledoduodenum with choledochoduo denostomy in our hospital. The results showed that there were no postoperative complications and death in our patients. A 2-year follow-up revealed no cholangitis,blind tubule syndrome and regenerative stones. The barium meal examinations showed that the barium refluxe into the binary trees was able to be removed within 6- 10 minutes. The main advantage of the operation is that it is simple and safe with big anastomotic calibre and no retention of bile and intestinal content. The mechanism,indications and notes for attention in the operation are disccused here in this article.
