Surgical correction of fingernail hawk deformation

目的分析鹰甲畸形的成因，探讨和改进鹰甲畸形的治疗方法。方法对1999年10月-2007年12月收治的8例（12指）鹰甲畸形患者，采用改良筋膜蒂V—Y提升皮瓣矫正指端皮肤挛缩，同时矫正合并其他的畸形。结果本组8例（12指）改良筋膜蒂V—Y提升皮瓣全部存活。术后5例9指获得随访，时间为6个月至3年，余3例失访。手指外形及颜色基本同正常手指，指端恢复到正常形态，指甲生长良好，原有手指运动及感觉功能保存良好，有弹性，耐寒冷，指端痛觉过敏消失，疼痛基本缓解。结论鹰甲畸形为指端外伤后处理不当所致，采用改良筋膜蒂V—Y提升皮瓣来纠正指腹皮肤挛缩，同时矫正合并其他畸形，可以获得满意的效果。

Objective To analyze the causes of hawk deformation of the fingernail, and to investigate and improve the treating methods of fingernail hawk deformation. Methods From October 1999 to December 2007, 8 cases （12 fingers） of fingernail hawk deformation were treated by modified fascial pedicle V-Y advancement flap to correct finger tip skin contracture and other combined deformities. Results All the modified V-Y advancement flaps in tiffs series of 8 cases （ 12 fingers） survived. Five cases 9 fingers were followed up postoperatively for 6 months to 3 years. The rest 3 cases were lost to follow up. The shapes and colors of these fingers were near normal. The fingertips restored normal morphology and the nails grew well, too. In addition, movement and sensation of the fingers were preserved. The fingers were flexible and cold-tolerant. Hyperalgesia of the fingertips disappeared and the pain was largely relieved. Conclusion The hawk deformation of fingernails is caused by improper treatment after finger tip injury. Correction of skin contracture at the pulp and other combined deformities by modified fascial pedicle V-Y advancement flap can lead to satisfactory results.
