应用ROC曲线评价血胱抑素C对窒息新生儿肾功能损害的诊断价值

Diagnostic value of analyzing serum cystatin C by ROC curve on renal dysfunction of asphyxia newborn

目的应用ROC曲线评价血胱抑素C(Cys C)水平对窒息新生儿肾功能损害的诊断价值。方法按Apgar评分标准将入选新生儿的血液标本分为正常对照组、轻度窒息和重度窒息三组。使用全自动生化分析仪对血清Cys C、尿素(BUN)、肌酐(Scr)进行检测。通过分析特征工作曲线(ROC)评价它们对诊断窒息新生儿肾功能损害的价值。结果①轻度窒息组血清Cys C浓度较对照组显著升高(P<0.01),重度窒息组血清Cys C浓度较轻度窒息组显著升高(P<0.01);②血清BUN、Scr、Cys C的ROC曲线下面积分别为0.782、0.881、0.957,差异有统计学意义(P<0.01)。结论血清Cys C可作为窒息新生儿肾功能损害的诊断指标,而且优于血BUN、Scr。

Objective To evaluate the diagnostic value of the serum cystatin C on renal dysfunction of asphyxia newborn by using the ROC curve. Methods According the standard of Apgar score,the sernm samples of newborns were divided into 3 group： the normal conteol group, the mild asphyxia group and severe asphyxia group. Automatic biochemical analyzer was used to measure the serum levels of Cys C, BUN and Scr. And we evaluate the diagnostic value of them on renal dysfunction of asphyxia newborn by analyzing the ROC curve. Results ①The concentration of serum Cys C in the mild asphyxia group was significantly higher than that in the normal control group（ P 〈 0. 01 ）, and it was significantly lower than that in the severe asphyxia group（ P 〈 0. 01 ）. ②The area under the ROC curve of BUN,Scr and Cys C in serum were 0. 782,0. 881 and 0. 957. There was a significant difference among them（P 〈0.01 ）. Conclusion Cys C in serum can be a diagnostic indicator of renal dysfunction in asphyxia newborn, and it is better than BUN and Scr.
