依贝沙坦逆转高血压左心室肥厚及其对QT离散度的影响

摘要 目的观察依贝沙坦降压疗效的同时,探讨其逆转左心室肥厚(LVH)及对QT离散度(QTd)的影响。方法观察组选择符合WHO分期诊断标准并首次诊治的轻、中度(I、Ⅱ期)高血压病患者共56例,给予依贝沙坦150mg/d治疗,治疗1周后,血压未降至正常范围者剂量加倍,达到理想的降压效果后,改维持量150mg/d以巩固治疗。观察时间为6个月。对照组40例为健康体检者。结果观察组QTd、校正QT离散度(QTcd)和导联校正后的QTcd(QTcd,lead-adjust)显著大于正常对照组(P<O.05);高血压伴LVH者QTd、QTcd和QTcd,1ead-adjust显著大于不伴LVH者(P<0.05)。结论原发性高血压无论伴LVH与否,QTd和QTcd均显著增加。QTcd与左心室质量指数(LVMI)呈正相关,依贝沙坦在有效降压的同时,还可逆转LVH并导致QTcd减小。 Objective To observe the blood pressure decreasing effect of Irbesartan and to analyze its effects in correcting LVH and QTd. Method 56 patients of light and moderate hypertension were chosen according to the timed diagnosis as the observation group,who are treated with Irbesartan at the dose of 150mg/d in the first week and at a doubled dose| in the second in case their b1ood pressure failed to decrease to the normal range. If satisfactory results were achieved,the dose was maintained at 150mg/d for consolidation and observation for 6 months;Another 40healthy persons were used as the control group. Results The QTd,QTcd and the QTcd after lead-adjust of the case of the observation group were remarkably higher than those of control group(P〈0.05);the QTd,QTcd and the QTcd after lead-adjust of patients of hypertension with LVH were remarkbaly higher than those of hypertension without LVH. Conclusion The QTd and QTcd of patients with hypertension undergo remarkable increase with or without LVH;the QTd and LVMI are correlsted;and Irbesartan is effective in decreasing blood pressure,correcting LVH as well as decreasing QTcd.
