疏肝健脾养心安神法治疗糖尿病抑郁障碍临床研究

Clinical Study on Liver-Coursing,Spleen-Strengthening,Heart-Nourishing and Mind-Calming Therapy for Diabetic Depression

目的观察糖郁汤治疗肝郁脾虚、心神不宁型糖尿病抑郁障碍的临床疗效。方法选择108例患者,共93例纳入分析。均维持原糖尿病基础及降糖药物治疗,随机分为试验组(糖郁汤,n=31)、对照组(左洛复,n=32)和空白组(n=30),疗程12周。观察各组治疗前后中医证候、汉密尔顿抑郁量表、汉密尔顿焦虑量表、糖尿病患者生存质量特异性量表、抗抑郁药副反应评定量表积分及血糖等变化。结果试验组中医证候疗效优于对照组(P<0.05);试验组与对照组抗抑郁疗效优于空白组(P<0.01),而两组间差异无统计学意义(P>0.05);对照组生存质量量表中心理健康积分高于试验组(P<0.05),其余4项差异均无统计学意义(P>0.05);试验组仅1例出现不良反应,对照组9例出现不良反应。结论糖郁汤可有效改善肝郁脾虚、心神不宁型糖尿病抑郁障碍患者临床症状,改善糖尿病抑郁及焦虑的不良情绪,有效提高糖尿病抑郁障碍患者生存质量;且具有良好的安全性。

Objective To observe the clinical efficacy of ＂Tangyu Decoction＂ in treating diabetic depression of liver-stagnation and spleen-deficiency syndrome. Methods Ninety cases out of 108 patients were enrolled, and were randomized into treatment group, control group, and blank group. All of the patients were given basic treatment of diabetes and sugar-reducing medicine, and the patients in the treatment group and the control group were treated with ＂Tangyu Decoction＂ and Zoloft respectively. The scores of TCM syndromes, Hamilton depression scale, Hamilton anxiety scale, diabetic patients＇ life quality scale, antidepressant sideeffect scale were observed, and blood sugar were detected. Results The TCM syndrome efficacy in the treatment group was superior to that in the control group （ P 〈0.05 ）, and there was a significant difference in anti-depression between the two groups （P 〉0.05 ）. The score of psychological health in the life quality scale in the control group was higher than that in the treatment group （P 〈0.05 ）. Side effects were found in one case in the treatment group and nine cases in the control group. Conclusion ＂Tangyu Decoction＂ is effective in improving TCM syndrome, depression and anxiety, and may safely promote the life quality of patients with diabetes of liver-stagnation and spleen-deficiency syndrome.
