儿童自体外周造血干细胞回输的护理

Nursing of Autologous Peripheral Blood Stem Cell Reinfusions in Children

目的总结儿童患者自体外周血造血干细胞回输的护理要点。方法35例恶性肿瘤患儿经大剂量化疗或化疗加放疗的预处理后,接受解冻后的冻存自体外周造血干细胞的回输。结果患儿共接受冻存自体造血干细胞回输70次,回输后出现了不良反应:血红蛋白尿54例次(77.1%)、头痛28例次(40.0%)、呕吐24例次(34.3%)、恶心17例次(24.3%)、心率失常11例次(15.7%)、腹痛8例次(11.4%)、腹泻2例次(2.8%)、发热1例(4.0%)。未发生心脏、肝、肾功能衰竭及休克等严重反应。结论儿童冻存自体外周造血干细胞回输易出现不良反应,护理人员应加强患儿的心理护理、专业的对症护理、预防不良反应,使回输安全有效进行。

Objective To summarize the nursing features of peripheral blood stem cell （PBSC） reinfusions in children. Methods A total of 35 children with malignant tumors received reinfusions of cryopreserved stem cells after undergoing high-dose chemotherapy or chemotherapy with radiation. Results The adverse reactions observed following 70 reinfusions included hemoglobinuria in 54 patients （77.1%）,headache in 28（40.0%）,nausea in 24（34.3%）,vomiting in 17（24.3%）, abdominal pain in 8（11.4%） ,diarrhea in 2（2.80%） and fever in 1（4.0%）. No shock or function failures in heart,liver and kidney occurred. Conclusion Adverse reactions occurred easily in children with re-infusions of cryopreserved peripheral blood stem cell. The procedure of reinfusions can be completed smoothlv with enhanced psychological nursing,symptomatic nursing and prevention of adverse reactions.
