摘要
目的:探讨高压氧治疗对颅脑外伤患者血糖以及治疗转归的影响。方法:146例颅脑外伤患者按GCS评分分为轻度组45例,中度组45例,重度组56例,每组再分别按常规治疗与配合HBO治疗2种方法随机分为A、B2个亚组,共治疗1~6个疗程。治疗前后比较各组血糖变化,并分析治疗效果。结果:①各组的A亚组与B亚组治疗前血糖分别为7.4±2.6mmol/L与7.8±3.1mmol/L,2亚组间比较差异无统计学意义;治疗后B亚组血糖明显低于A亚组(5.6±0.9mmol/L与6.1±0.9mmol/L,P<0.05)。各组中GCS评分高的患者血糖低于评分低的患者,其轻度组预后较好。③B亚组预后较A亚组好。结论:HBO对患者血糖的稳定及预后改善有显著帮助。
Objective: To study the effects of hyperbaric oxygen (HBO) therapy on blood sugar and prognosis of patients with craniocerebral trauma. Methods: One hundred and forty-six cases were divided into 3 groups according to GCS scores (mild group, n= 45; moderate group, n = 45; severe group, n = 56), and then randomized into HBO subgroup and control subgroup in every group. There were a total of 1 to 6 treatment courses. Levels of blood sugar were determined in 24 h after admission, and before and after treatment. Results: Blood sugar levels in control and HBO subgroups were 7.4±2.6 and 7.8±3.1 mmol/L respectively before treatment. After treatment they were reduced to 6.1±0.9 and 5.6±0.9 mmol/L respectively (P〈0.05). GCS scores were associated with severity and prognosis of craniocerebral trauma, and also possibly with the levels of blood sugar. The level of blood sugar in mild group was lower than in severe group. The prognosis in HBO subgroup was more satisfactory than in control subgroup. Conclusion: HBO therapy can reduce the level of blood sugar and improve the prognosis of the patients with craniocerebral trauma.
