糖胰康治疗成人自身免疫性糖尿病的临床研究

Clinical Study on Tangyikang for Adults Autoimmune Diabetes Mellitus

目的:研究糖胰康对成人自身免疫性糖尿病(LADA)的疗效与机理。方法:将54例胰岛素治疗中的LADA患者随机分为两组,治疗组29例加服糖胰康治疗,对照组25例继续胰岛素治疗,疗程3个月。观察治疗前后两组的糖化血红蛋白(HbA1c)、胰岛素释放指数(HOMA-IS)、胰岛素抵抗指数(HOMA-IR)、超氧化物歧化酶(SOD)、丙二醛(MDA)、抗氧化比值(AOR)和胰岛素用量(INSD)。结果:治疗后治疗组的HOMA-IS、SOD、AOR高于对照组,MDA、INSD低于对照组,与对照组比较均有统计学差异(P<0.05);HbA1c、HOMA-IR与对照组比较无统计学差异(P>0.05)。多元逐步回归分析显示:入组时FPG、FINS和处理组别进入回归方程,3个变量的标准回归系数分别为-0.584、0.324、0.392,P值均<0.0001,提示处理组别因素(即是否服用糖胰康)对HOMA-IS影响最大。结论:糖胰康可改善LADA患者的胰岛β细胞功能,其机理与抑制体内氧化应激反应有关。

Objective：To observe the curative effect of Chinese herbal medicine：Tangyikang Decoction on autoimmune diabetes mellitus in adults（LADA） ,explore the mechanism and offer the scientific basis for the further improvement and clinic application of Tangyikang decoction. Methods ：54 cases of insulin treatment in LADA patients were randomly divided into two groups of 29 cases and the treatment of Tangyikang plus insulin,25 cases of the control group continued to insulin treatment for 3 months. Before and after the treatment the two groups of glycosylatedhemoglobin（HbAlc）. Results ： the release of insulin index （ HOMA - IS）, insulin resistance index （ HOMA - IR）, superoxide dismutase （ SOD ）, MDA, anti - oxidation ratio （AOR） and insulin dosage （INSD）. Results ： The results of the treatment group, the HOMA - IS, SOD, AOR higher,and MDA, INSD lower than the control group, compared with the control group had significant difference （P 〈 0. 05 ） ; HbA1 c, HOMA - IR compared with the control group, there was no difference （ P 〉 0. 05 ）. Multiple regression analysis showed that： the group at FPG, FINS and processing groups into the regression equation, the three return to the standard variable coefficients were - 0. 584,0. 324,0. 392, P 〈 0. 0001, suggested that treatment group factors （ that is, whether taking Tangyikang）HOMA -IS the greatest impact. Conclusion ：Tangyikang could improve the β -cell function of LADA patients. Its mechanism was possibly inhibited oxidative stress on the body.
