从孙一奎医案谈五脏阴阳辨证的临床应用

Discussion on the Clinical Use of the Differentiation of Symptoms and Signs with the Theory of YIN and YANG in Five Viscera Based on SUN Yi-kui Case

五脏阴阳辨证渊源于《黄帝内经》,是一种集唯物论和辩证法思想于一体的医学思辨模式,在中医学理论体系中占有十分重要的地位。自《黄帝内经》以后,五脏阴阳辨证作为一种重要的辨证思维模式一直为历代医家所重视。举明代医家孙一奎医案为例,通过分析医案的辨证立法、处方用药,说明五脏阴阳辨证在中医临床诊疗方面的指导意义。即通过加强和调整五脏的功能活动,使各脏腑的功能状态恢复到"阴平阳秘"的理想水平,从而增强机体的自稳调节能力和抗病敌邪能力,正是中医临床治疗的诀窍所在。

Differentiation of symptoms and signs with the theory of YIN and YANG in five viscera started from Huangdi Neijing. It is a kind of way in medicine of thinking and diagnosis that combines materialism and idalectics and plays the important role in the theoretical system of traditional Chinese medicine. Since Neijing, as one of the important thinking and diagnosis ways ,differentiation of symptoms and signs with the theory of YIN and YANG in five viscera has all the time been emphasized by many generations of doctors. With the example of SUN Yi-kui（ in Ming Dynasty）Medical Record,the guidance shown by differentiation of symptoms and signs with the theory of YIN and YANG in five viscera in clinical treatment of traditional Chinese medicine was elucidated through the analysis of differentiation of symptoms and signs, treatment establishment and prescription in the case. That is, via the function of five viscera enforcement and adjustment, all the functions of Zang-Fu recover to the ideal status of YIN and YANG in a relative equilibrium, which further promotes the abilities of body self-stability regulation and disease-preventiun. This is the trick of traditional Chinese medicine in clinic.
