宫颈葡萄胎的临床特点及诊治(附1例病例报告及文献复习)

Clinic characteristic,diagnostic method and treatment of cervical hydatidiform mole

目的:探讨宫颈葡萄胎的临床特点和诊治方案。方法:报告1例宫颈葡萄胎的临床资料,总结国内外相关个案报道,对宫颈葡萄胎的临床表现、诊断和治疗进行分析。结果:宫颈葡萄胎发病率低,临床表现为阴道出血时间长、量多,腹痛不明显,宫颈形状异常,血β-人绒毛膜促性腺激素(β-HCG)较宫颈妊娠高,B超提示宫颈部位葡萄胎改变等,治疗上以5-氟脲嘧啶为主的化疗,效果良好。结论:宫颈葡萄胎临床罕见,临床表现特殊,血HCG和B超可辅助诊断,治疗应选择以5-氟脲嘧啶为主的化疗。

Objective：To reseach the clinic characteristic,the method of diagnosis and treatment of cervical hydatidiform mole.Methods：The clinical data of the present case was analyzed combining with the other cases reported in the world.The clinical manifestation,diagnosis and treatment were reviewed.Results：Cervical hydatidiform mole showed extremely low incidence rate,long period and massive vaginal bleeding,no evident abdominal pain,abnormal shape of the uterine cervix,higher blood β-human chorionic gonadotrophin（β-HCG） than cervical pregnancy and specific image of B type ultrasound.About treatment,the operation risk is very high and chemotherapy with 5-flurouracil showed good therapeutic effect.Conclusion：Cervical hydatidiform mole is a rare disease and has special clinical manifestation.Blood β-HCG and B type ultrasound can help to diagnose.Chemotherapy with 5-flurouracil has satisfactory effect.
