应用腹腔镜及开腹手术治疗197例宫外孕临床分析

Clinical Analysis of Using Laparoscopes to Diagnose and Treat Ectopic Pregnancy by Comparing with Laparotomy

目的:探讨腹腔镜手术与剖腹手术诊断治疗宫外孕的优缺点。方法:回顾性分析医院2002年8月至2008年6月收治的197例宫外孕患者,比较两组手术时间、出血量、并发症、术后住院日及术后妊娠等情况。结果:103例成功实施腹腔镜手术,手术时间较剖腹手术时间无明显差异,腹腔镜手术较开腹手术在减少出血量、并发症,减少住院时间,尽快康复有明显优势,两者差异有显著性(P<0.01)。结论:急诊进行腹腔镜探查宫外孕患者手术安全有效,并发症少等优点,具有诊断和治疗的双重功效,值得推广。

Objective： To explore the value of laparoscope and laparotomy in diagnosis and treatment of ectopic pregnancy. Method： 197 patients with ectopic pregnancy treated with laparoscope and laparotomy were retrospectively analyzed in our research from August of 2002 to June of 2008. One hundred and fifteen （115） cases were in the laparoscopic group,while eighty two （82） in laparotomy group. Parameters including operating time, the haemorrhage volume, complications and fallopian tube potency were collected for comparison. Result： One hundred and three （ 103 ） patients operated successful, 115 patients treated with laparoseopy. The operation time of laparoscopy was no longer than that of laparotomy. Laparoseopie treatment of ectopie pregnancy patients offers major benefits superior to laparotomy in terms of less blood loss,less complication, shorter duration of hospital stay and convalescence. Conclusion： Laparoseopic procedure was safe, effective and less complication in management of ectopie pregnancy patients, which can serve to diagnose and treat ectopic pregnancy.
