准分子激光原位角膜磨镶术术前、术后角膜厚度与眼压的关系 被引量:1

THE RELATION BETWEEN CORNEAL CENTRAL THICKNESS AND INTRAOCULAR PRESSURE PRE AND POST OPERATION OF EXCIMER LASER IN SITU KERATOMILEUSIS

摘要 目的评价准分子激光原位角膜磨镶术(LASIK)术前、术后角膜厚度与眼压间变化的关系。方法242例(484只眼)分别于LASIK术前、术后测量角膜厚度、眼压,并比较角膜中央厚度改变与眼压计读数差异间的关系。对所有数据进行t检验和相关分析。结果LASIK术前角膜中央厚度为(542.13±30.36)μm,术后为(462.73±33.28)μm,减少了(79.40±29.07)μm,差异有统计学意义(t=41.051;p<0.001)。LASIK术前眼压为(14.72±2.67)mmHg(1 mmHg=0.133kPa),术后为(10.63±2.56)mmHg,其差值为(4.09±2.82)mmHg,有统计学意义(t=18.003;p<0.001)。LASIK术前、术后角膜中央厚度与眼压测量值均呈高度正相关(r=0.336,p<0.01;r=0.262,p<0.01),即角膜厚度变薄,眼压测量值降低。结论LASIK术后眼压测量值随角膜中央厚度的减少而降低。角膜中央厚度的改变是眼压值下降的主要因素。接受LASIK手术者中部份可能存在有青光眼因素,故应关注此类术后眼压"正常"者,以避免对青光眼的漏诊。 Objective To evaluate the relationship and significance between corneal thickness( CCT) and intra ocular pressure(lOP) pre and post operation of excimcr laser in situ keratomileusis(Lasik). Methods The CCT and IOP were measured before and after Lasik in all of the 484 eyes of 242 cases. The change of lOP readings and its relationship with CCT were compared and analyzed by t - test and correlation analysis. Results The CCT before and after Lasik was ( 542. 13 ± 30. 36 )um and (462. 73 ± 33.28 ) um respectively. The reduction of CCT was (79. 40 ± 29.07 ) um. There was statistic significance in the difference ( t = 41. 051 ;p 〈 0. 001 ). The IOP readings before and after Lasik was( 14. 72 ± 2. 67 ) mmHg ( 1 mmHg = 0. 133 kpa) and ( 10. 63 ± 2. 56) mmHg. The reduction was (4. 09 ±2. 82) mmHg. There was statistic significant in IOP pre and postoperatively( t = = 18. 003 ,p 〈0. 001 ). The CCT and IOP readings before and after Lasik were highly positive correlatian(r =0. 336,p 〈0. 01 ;r =0. 262,p 〈0. 01 ). That is IOP readings were decreased as CCT became thiner after Lasik. Conclusion The IOP readings after Lasik decrease till CCT decline. The CCT change is the major factor of IOP reduction. A part of patients who underwent Lasik possibly has glaucoma factors. In order to avoid failing diagnosis of glaucoma we must pay more attention to these persons with the" normal IOP" who have undergone Lasik.
