探讨肺部感染患者肺泡灌洗液中T淋巴细胞亚群的临床意义

Clinical Significance of T-lymphocyte Subsets in the Lavage Fluid and Peripheral Blood from ICU Patients with Pulmonary Infection

目的探讨肺部染患者外周血及肺泡灌洗液中T淋巴细胞亚群的变化特点及其临床意义。方法采用流式细胞术同时检测22例ICU肺部感染患者和22例志愿者的肺泡灌洗液及外周血中T淋巴细胞亚群。结果感肺组泡灌洗液中CD3＋比例（61．69％±18．69％），较正常对照（57．64％±13．84％）变化不明显，CD4＋比例（25．04％±14．24％），较正常对照（33．01％±11．94％）显著降低，CD8＋比例（40．40％±13．53％），较正常对照（30．63％±12．32％）显著增高，CD4＋／CD8＋比值（0．69％±0．34％），较正常对照显著降低。而感染组外周血中CD3＋、CD4＋、CD8＋均下降。CD4＋／CD8＋比值无明显变化。结论肺泡灌洗液中淋巴细胞的检测能更好地反映患者肺部感染的情况，对临床诊断及治疗有较大帮助。

Objective To explore the change features and clinical significance of T-lymphocyte subsets in bronchoalveolar lavage fluid and peripheral blood（BALF） of ICU patients with pulmonary infection.Method The T- lymphocyte subsets in bronchoalveolar lavage fluid and peripheral blood of 22 cases of pulmonary infection in ICU patients and 22 healthy volunteers is detected using flow cytometry.Results The proportion ofCD3 ＋ in the BALF of the infected group is （61.69% ± 18.69%）,compared with the normal control （57.64% ± 1.84%） did not change signifieantly;CD4＋is （25.04% ±14.24%）,compared with the normal control（33.01% ± 11.94%）,a significant reduction;CD8＋ is （40.40% ± 13.53%）,compared with the normal control （30.63% ± 12.32%） was significantly higher;CD4＋/CD8 ＋ is （0.69%±0.34%）,compared with the normal control have significantly decreased.CD3＋, CD4＋,CD8＋ in the infected blood decreased,and the CD4＋/CD8＋ratio remained unchanged.Conclusion The T- lymphocyte subsets test in the lavage fluid can better reflect the situation in patients with pulmonary infection,and it is more help for the clinical diagnosis and treatment.
