大块骨溶解症

Massive osteolysis

目的探讨大块骨溶解症的临床表现,病理学、影像学特点及诊断治疗方法。方法报道1例大块骨溶解症病例,结合文献总结本病的临床表现、病理学、影像学特点及诊断治疗方法。结果大块骨溶解症的临床表现、病理结果及X线检查常不一致,极易误诊、漏诊;临床表现多无特异性;病理学亦无特异性,早期以血管增生为主,后期以纤维组织增生为主;X线表现为大量骨质溶解吸收但无骨质增生硬化,无骨膜反应,无瘤骨形成,早期边界模糊,后期界清、锐利。结论大块骨溶解症早期诊断困难,必须综合临床表现、病理及X线检查等结果;应根据本病活动期或静止期,给予对症治疗,长期随访。

Objective To explore the clinical manifestations, pathological features, radiological findings, diagnosis and treatment of the massive. Methods To report a typical case, and sum up the clinical manifestations, pathological features, radiological findings, diagnosis and treatment combined related literature. Results There are many of inconsistencies amony the clinical manifestations, pathological features and radiological findings. So massive osteolysis is easy to be missed diagnosed and misdiagnosed. The clinical manifestations and pathological features are non- specific. The x-ray manifestations is that there are mass of osteolysis, no hyperostosis osteosclerosis, no periosteareaction, no tumor bone in sick bones. Conclusions The early diagnosis of massive osteolysis is difficult. We must analysis comprehensivly the clinical manifestations, pathological features and radiological findings, and eliminate tumors, inflammations and tuberculosis. Different treatments should to be given in different stages.And long-term follow-up should to be provided to patients.
