高血压病左室构型与QT离散度关系的研究

Study of relationship between left ventricular pattern and QT dispersion in patients with primary hypertension

目的研究原发性高血压心脏超声构型的分型及其与QT离散度的关系。方法选102例原发性高血压患者采用超声心动图(Ganau标准)将患者分为:①正常构型组:左室质量指数及相对室壁厚度均在正常范围。②向心性重构组:左室质量指数正常,但相对室壁厚度增加。③向心性肥厚组:左室质量指数与相对室壁厚度均增加。④离心性肥厚组:左室质量指数增加但相对室壁厚度正常。⑤正常对照组:匹配50名年龄相当的健康查体者作为对照组。记录其12导联同步心电图,经扫描仪以150分辨率扫描入计算机,利用图像处理软件photo-shop7.0版的测量工具+目测判断,对心电图资料进行QT间期的测量,计算出QTd。结果QTd在高血压患者B、C、D组较正常对照组显著升高(P<0.05或0.01),C、D组较A、B组显著升高(P<0.05或0.01)。结论高血压病患者心电图QT离散度增大,在向心性肥厚和离心性肥厚这两个构型中表现更为明显。

Objective： To study the relation between the left ventricular pattern （LVP） and the change of QT dispersion （QTd） , in patients with primary hypertensive. Methods： 102 patients with primary hypertension were randomly selected depending on the results detected by color Doppler echocardiography . All these patients were dividied into four groups according to the standards of Ganau, the normal left ventricle group, the concentric remodeling group, the concentric hypertrophy group and the eccentric hypertrophy group. And 50 control persons were monitored. Each of all subjects was recorded 12-lead synchronously electrocardiogram, which would be scanned into the computer with the distinguishing rate of 150. To measure the QT interval and to calculate theQTd, each electrocardiogram was measured with the measure tool of the picture handles software, photoshop 7.0 version, and with the judgement of eyes. Results ： QT dispersions of QTd in group B, group C and group D were significantly higher than those of the normal control group （ P 〈 0. 05 or P 〈 0.01 ）. QTd in group C and group D was significantly higher than those of group A and group B （P 〈0. 05 or P 〈0. 01 ）. QTd in patients with primary hypertension increased gradually in group A, group B, group C and group D. Conclusion： There is close correlation between the left ventricular remodeling and QT dispersion in patients with primary hypertension.
