中西医结合治疗老年性慢性阻塞性肺疾病急性加重期疗效观察

Clmical Observation on Chronic Obstructive Pulmonary Disease in Acute Exacerbation Stage by the Treatment of Traelitional Chinese Medicine and Western Medicine

目的:观察中西医结合治疗老年性慢性阻塞性肺疾病急性加重期的肺功能及其临床疗效。方法:将50例COPD急性发作患者随机分为2组,中西医结合治疗组25例在予以西医常规治疗同时加服加味三子养亲汤治疗;对照组25例采用西医常规治疗。观察两组患者肺功能及临床症状、体征改善情况。结果:经过1个疗程的治疗后,两组治疗后第1 s呼出气量(FEV1)、用力肺活量(FVC)均较治疗前均有改善,中西医结合治疗组治疗后第1 s呼出气量(FEV1)、用力肺活量(FVC)改善与对照组比较有显著性差异(P<0.05)。中西医结合治疗组临床控制10例,显效11例,好转3例,无效1例,总有效率为96.0%。对照组临床控制5例,显效7例,好转6例,无效7例,总有效率72.0%。两组总有效率比较,差异有显著性意义(P<0.05)。中西医结合治疗组临床症状及体征改善优于对照组(P<0.05)。结论:中西医结合治疗慢性阻塞性肺疾病急性发作具有良好的临床疗效,并优于西医常规治疗。

Objectives： To observe the therapeutic effect of combined treatment of Chinese and western medicine on pulmonary function of chronic obstructive pulmonary diseases（COPD） in acute exacerbation stage.Methods： 50 acute COPD patients were randomly divided into 2 groups as Chinese and western medicine treatment group（＂CWMT＂ group,n=25,given routine western medical treatment and modified Sanzi Yangqin Decoction） and control group（n=25,received routine western medical treatment）.Improvement of pulmonary function,clinical symptoms and signs of patients in both groups was observed.Results： After one course of treatment,both groups showed improvement in forced expiratory volume in one second（FEV1） and forced vital capacity（FVC）.The CWMT group demonstrated significant differences in FEV1 and FVC comparing with that of control group（P〈0.05）.10 cases were clinically alleviated in the CWMT group along with 11 cases effective,3 cases improved and 1 case ineffective,and the total effective rate was 96.0%.Meantime,the control group achieved 5 cases with clinical alleviation,7 cases with effectiveness,6 cases with improvement and 7 cases inefficacy,and 72.0% total effective rate.The comparison of total effective rate of the two groups showed significant differences（P〈0.05）.Better improvement in clinical symptoms and signs was observed in CWMT group than in control group（P〈0.05）.Conclusion： The combined treatment of Chinese and western medicine has a favorable clinical therapeutic effect in treating chronic obstructive pulmonary diseases in acute exacerbation stage,with a superior effect to that of routine western medical treatment
