国产机械吻合器在食管癌和贲门癌术中的应用体会

Experience in application of the domestically produced mechanical anastomat at esophageal cardiac carcinoma

目的评价国产机械吻合器在食管癌和贲门癌手术中的应用。方法选取自2002年10月至2008年8月期间,采用国产圆形管状吻合器行食管、贲门癌切除吻合术158例,其中男103例,女55例,年龄44～79岁。食管癌140例,贲门癌18例。食管胃颈部吻合7例,胸顶吻合81例,弓上吻合43例,弓下吻合27例。结果术后出现吻合口瘘3例(1.9%),吻合口狭窄6例(3.8%),吻合口出血2例(1.2%),术中吻合出现机械故障2例(1.2%)。结论国产机械吻合器在食管贲门癌手术中是安全省时的,可反复使用,价格低廉,能满足基层医院和偏远地区大部分患者的需要。

Objective To appraisal the application at esophageal cardiac carcinoma surgery using the domestically produced mechanical anastomat. Methods From October 2002 to August 2008,158 patients with esophageal cardiac carcinoma using the domestically produced circular tubular anastomat in surgery, in which male 103 cases ,female 55 cases, Age 44-79 year old. Esophagus cancer 140 cases, cardiac opening of stomach cancer 18 cases. Anastomosis at cervix by esophagus-stomach 7 cases, at top of the chest 81 cases,on the A bow 43 cases, under the A bow 27 cases. Results After surgery, anastomotic fistula 3 cases （ 1.9% ）, anastomotic narrow 6 cases（3.8% ） ,anastomotic hemorrhage 2 cases（ 1.2% ）. In the surgery appliances breakdown 2 cases （1.2%）. Conclusion The domestically produced mechanical anastomat in the surgery of esophagus cardiac carcinoma is safe time-saving, may use repeatedly, the price is inexpensive, can satisfy the majority of patient＇s need in basic unit hospital and the remote districts.
