肝癌介入治疗靶血管损伤原因及防治探讨

Discussion of the Causes Prevention and Cure of the Injured Target Vessel in the Patients with Arterial Chemoembolization for Hepatocellular Carcinoma

目的探讨肝癌肝动脉化疗栓塞(TACE)术中肝动脉及其分支损伤原因及其预防、处理。资料与方法2007年5月至2007年7月行肝TACE术患者164例,观察并分析术中肝动脉及其分支损伤情况。结果164例中发生血管损伤者57例(34.76%),其中47例(28.66%)发生血管痉挛,8例(4.88%)发生血管穿孔,2例(1.22%)发生内膜撕裂。结论肝癌TACE血管损伤发生与术者操作、导管头位置、患者血管条件有关,应根据不同情况积极预防、处理。

Objective To discuss the causes, prevention and cure of the injured hepatic artery which undergone the therapy of TACE for primary hepatic cancer. Materials and Methods TACE was performed in 164 patients with PHC during May-July 2007. Results The injured hepatic arteries were found in 57 patients （34.76%）. In the cases, arterial spasm in 47 （28.66%）, arterial tear in 8 （4.88%）, arterial dissection in 2 （ 1. 22% ）. Conclusion The operation of operator, the position of catheter and the situation of artery were involved in hepatic artery injury.
