成人心脏超声探头在胎儿先天性心脏病诊断中的应用

Application of adult heart ultrasonic probe in diagnosis of fetal congenital heart diseases

目的探讨成人心脏超声探头在胎儿先天性心脏病(简称先心病)诊断中的应用价值。方法分析19例产前超声诊断、产后超声复查和胎儿引产后尸解证实的胎儿先心病病例资料。结果应用成人心脏超声探头探查,诊断胎儿①复杂性先心病14例,占73.6%(14/19),包括完全性心内膜垫缺损、大动脉转位,单心房、单心室、右室双出口、右室发育不良、瓣膜狭窄或闭锁、永存动脉干及法洛四联症;②单纯室间隔缺损(VSD)4例;③房间隔膨胀瘤并房间隔缺损(ASD)1例。结论成人心脏超声探头可应用于胎儿心脏的检查,能清楚显示心脏和大血管的血流信号,大大提高胎儿先心病的检出率。

Objective To assess application value of adult heart ultrasonic probe in diagnosis of fetal congenital heart diseases. Methods The ultrasonic images of 19 fetus with congenital heart diseases were analyzed,and they were detected by prenatal ultrasound,reexamined by ultrasound after parturition, and confirmed by fetal autopsy. Results （1） There were 14 cases of complicated fetal congenital heart diseases （73.6%, 14/19）, including complete endocardial cushion defect, transposition of the great arteries, single atrium, single ventricle, double outlet of right ventricle, hypoplastic right heart syndrome, stmitness or atresia of valve, truneus arteriosus persisteus and tetralogy of Fallot. （2） There were 4 eases of simple ventricular septal defect （VSD）. （3） There was 1 case of atrial septal aneurysm expansion with atrial septal defects（ASD）. Conclusion Adult heart ultrasonic probe can be used in diagnosis of fetal cardiac diseases, it can clearly display blood flow signals of heart and great blood vessel, and also can improve the detection rate of fetal cardiovascular abnormality.
