上颌智齿致张口受限14例临床总结

14 Cases Report for Limited Mouth Opening Caused by Upper Wisdom Teeth

目的:分析上颌智齿致张口受限的原因及临床特点。方法:对14例上颌智齿致张口受限病例的病史、临床症状、疗效进行回顾性分析。结果:14例患者拔除上颌智齿后2周复查,10例张口度恢复正常,其余4例1个月后复查张口度在3cm以上。结论:上颌智齿致张口受限是牙体对颊部组织的机械刺激,局部黏膜角化、软组织纤维化,溃疡形成,肉芽组织增生,咀嚼肌痉挛所致。拔除上颌智齿消除机械刺激是最有效的治疗方法。

Objective： To analyse the causes and clinical features of limited mouth opening related to upper wisdom teeth. Methods： Case history, clinical symptoms and therapeutic effect of 14 patients were analysed retrospectively. Results： After extraction of upper wisdom teeth, 10 cases of limited mouth opening became normalised within 2 weeks. One month later, the width of mouth opening reached to more than 3.0 cm. in the other 4 cases. Conclusion： Upper wisdom teeth caused mechanical stimulation, local keratinized mucosa, soft tissue fibrosis, ulcer formation,granulation tissue hyperplasia and masticatory muscle spasm, resulted in the limitation of mouth opening. Upper wisdom teeth extraction is the most effective therapy.
