摘要
目的:探讨上颌阻生尖牙的手术导萌和正畸矫治各种因素对治疗的影响。方法:21例上颌埋伏阻生尖牙的病例,通过CT三维重建技术对上颌阻生尖牙进行定位,采取外科翻瓣导萌术,在其唇(腭)侧开窗,在尖牙上粘接托槽,通过正畸方法进行牵引导萌。结果:CT三维重建图像能立体地显示上颌阻生尖牙的部位,移位情况,萌出方向及与周围组织的关系,提供良好的阻力分析。经准确定位,上颌阻生尖牙唇(腭)侧翻瓣导萌,正畸牵引21例,经过1~2年的矫治均取得满意的矫治效果。结论:CT三维重建处理技术能够对阻生尖牙立体直观地提供临床所需信息,帮助临床医师制定治疗计划及手术方案。唇侧阻生比腭侧阻生矫治时间要快,年龄偏大的患者矫治速度要慢。
Objective:To analyze the influence of various possible factors to orthodontic treatment of impacted maxillary canines. Methods:21 cases with impacted maxillary canines were chosen and examined with 3D-reconstruction of spiral CT. Maxillary labial or palatal impacted canines were extracted through flap operation and orthodontic techniques. Results:The 3D-reconstruction of spiral CT can clearly display the position, movement, erupting direction of the impacted canine and its relation with the surrounding tissues. After precise positioning,flap operation and orthodontic traction,21 cases all achieved desirable effects during 1-2 years of treatment. Conclusion:The method of 3D-reconstruction of spiral CT can display the dental 3D image clearly,provide the information of the impacted tooth, and can help orthodontists in making treatment designs and operation plans. Palatal impacted has longer treat time than labial impacted canines in orthodontic traction. Adult patients have longer time than younger patients in the duration of treatment.
