摘要 目的免疫组化观察CD68在腰椎间盘MRI高信号区中的表达。方法对26例下腰痛患者MRI的T2像HIZ区椎间盘组织和4例脊柱侧凸椎间盘组织进行CD68免疫组化染色观察。结果在HIZ区表达不等数量的CD68免疫阳性细胞,HIZ区CD68免疫阳性细胞明显多于HIZ区周边纤维环组织和对照组(P<0.05)。结论腰痛患者腰椎间盘MRI的HIZ区中表达大量CD68免疫阳性细胞,表明HIZ作为致痛椎间盘的一个标志物存在炎症反应的机制基础。 Objective To observe the immunohistochemistry for CD68 in the high intensity zone (HIZ) on MRI within the annulus fibrosas of discs in patients with low back pain. Methods Twenty-six disc samples from posterior annulus fibrosus of lumbar intervertebral disc in HIZ areas on MRI from patients with low back pain and control samples from scoliosis patients ( n =4) were harvested during surgery. Discs were immunostained with the use of antibodies to TNF-α and immunostained cells were counted. Results Higher CD68 expression in posterior annulus fibrosns in ttlZ areas on MRI were observed compared with normal disc on MRI. The number of CD68 immunoreactive cells in HIZ was significantly higher than that in annulus fibrosus arounding HIZ ( P 〈 0.05). Conclusion High CD68 expression in HIZ on MRI contained within the anulus of discs from patients with low back pain may indicate the inflammation mechanism for HIZ as specificity sign of painful disc on MRI.
