甲氰咪胍联合曲唑酮治疗消化性溃疡对照研究

A controlled study of cimitidine plus trazodone in the treatment of peptic ulcer

目的探讨甲氰咪胍联合曲唑酮治疗消化性溃疡的临床疗效。方法将96例消化性溃疡患者随机分为两组，因疗程不足脱落6例；研究组47例，对照组43例，两组均常规应用甲氰咪胍治疗，研究组联合曲唑酮治疗，观察4w。于治疗前及治疗4w末，采用症状自评量表评定两组患者的心理健康状况；治疗1w内依据溃疡疼痛程度评定标准判定疼痛缓解程度；治疗结束后1w内复查胃镜或消化道钡餐判定溃疡愈合情况。结果研究组溃疡愈合率（80．85％）显著高于对照组（55．81％）（Χ^2=6．66，P〈0．05）；缓解疼痛的速度显著快于对照组（P〈0．05）；治疗后症状自评量表躯体化、强迫、抑郁、焦虑、人际关系、敌对和恐怖因：子分均较治疗前有显著下降（P〈0．01），而对照组仅抑郁、焦虑因子分显著降低（P〈0．01）；研究组躯体化、强迫、抑郁、焦虑因子分显著低于对照组（P〈0．01）。结论甲氰咪胍联合曲唑酮治疗能显著改善消化性溃疡患者的心理健康状况，快速缓解疼痛症状，提高溃疡愈合率。

Objective To explore the efficacy of cimitidine plus trazodone in the treatment of peptic ulcer. Methods 90 peptic ulcer patients were randomly divided into research（n=47） and control group（n=43）, 6 dropped out because of course of therapy insufficient. Two groups received routine cimitidine treatment and the reaearch group was plus trazodone for 4 weeks. Mental health status was assessed with the Symptom Checklist-90（SCL-90） before treatment and at the end of 4 week treatment; ulcer healing states were determined according to re-gastroscopy or digestive tract BaM within the 1st week after treatment. Results Ulcer healing rate was significantly higher in the research（80.85 %） than in the control group（55.81 %）（Χ^2 =6.66,P〈0.05） ;speed of relieving pain was significantly faster in the research than in the control group （P〈0.05） ; according to the SCL-90, after treatment such factors＇ score as somatization, compulsion, depression, anxiety, interpersonal relationship, hostility and phobia reduced more significantly compared with pretreatment in the research（P〈0.01）, in the control so did depression and anxiety only（P〈0.01）; scores of somatization,compulsion,depression and anxiety were significantly lower in the research than in the control group（P〈0. 01）. Conclusion Cimitidine combined with trazodone can significantly improve mental health status of patients with peptic ulcer, relieve pain symptoms quickly and inereas.e ulcer health rate.
