经皮主动脉内覆膜支架技术治疗DeBakey Ⅲ型主动脉夹层

Treatment of Percutaneous Endovascular Stent-graft for DeBakey Ⅲ Aortic Dissection

目的:探讨经皮主动脉内覆膜支架技术治疗DeBakeyⅢ型主动脉夹层的疗效。方法:回顾性分析采用经皮主动脉内覆膜支架技术治疗DeBakeyⅢ型主动脉夹层患者64例的临床资料。结果:64例手术均成功。术后48h内,1例出现TIA1次,1例出现上消化道出血,2例出现脑梗死,1例出现肾功能损害并于10d后恢复。术后追踪随访6～12个月,无截瘫发生,胸背剧痛等症状均消失。结论:经皮主动脉内覆膜支架技术治疗DeBakeyⅢ型主动脉夹层近期效果满意,但需合理选择适应证,术前、后应合理控制血压。

Objective： To investigate the therapeutic effect of percutaneous endovascular stent-graft treatment for DeBakey Ⅲ aortic dissection. Methods： Clinic data of 64 cases with DeBakey Ⅲ aortic dissection underwent percutaneous endovascular stent-graft treatment were analyzed retrospectively. Results： All the 64 operations were successful. Within 48 hours after the operation, 1 case appeared transient ischemic attack, 1 case appeared upper gas- trointestinal hemorrhage, 2 cases appeared cerebral infarction, 1 case appeared renal function damage and were re covered in 10 days. During 6 to 12 months＇ post-operative follow up, no paraplegia occurred and the symptoms such as backache and lumbago disappeared. Conclusions： Short-term effect of percutaneous endovascular stent graft for DeBakey Ili aortic dissection is satisfactory, however, proper indications should be selected and blood pressure should be appropriately controlled before and after the operation.
