MRU对非结石性尿路梗阻性病变的诊断价值

摘要 目的本文通过对37例非结石性病变引起的尿路梗阻的MRU检查进行研究分析,充分体现了MRU的技术优势的价值。方法本组37例尿路梗阻病人,男性20例,女性17例,平均年龄45.5岁,所有病例全部行IVU与MRU检查,其中5例又做了CT检查,2例做了逆行肾盂造影,其中33例经手术证实。结果先天发育畸形10例(肾盂输尿管结合部狭窄2例;重复畸形6例,1例输尿管囊肿,脐尿管囊肿1例),内源性肿瘤14例(肾癌、肾盂及膀胱癌各4例;输尿管癌2例),外源性肿瘤6例(卵巢癌2例。巨大神经纤维瘤1例,转移性腺癌1例,结肠癌2例),炎症性病变7例(前列腺增生4例,结核2例,血块1例)。结论MRU是一种安全无创,无需对比剂的检查手段,充分补充IVU不显影的不足,梗阻部位定位准确,明显优于IVU,定性需结合CT及MR扫描。 Objective To discuss the diagnostic value of MR urography (MRU) in patients with urinary obstruction because of noncalculous lesion. Methods 37 cases with noncalculous urinary obstruction underwent MRU and 1VU, including 20 cases of male and 17 of female. Among them, CT was carried out in 5 cases and retrograde urography in 2 cases, 33 cases were confh'med by surgery. Results In 37 patients, result was included as followed: 10 cases of congenital developmental malformation, 14 cases of endogenous tumor, 6 cases of ectogenesis tumor and 7 cases of inflammation lesion. Conclusion MRU is a safe noninvasive imaging technique without contrast medium, which can complement for inadequacy of nonvisualization in IVU. Combined to CT and MRI scan, it surpasses in determining the nature to IVU, providing accurate location of the urinary obstruction.
