杓会厌襞血管瘤的手术治疗

Surgical treatment of aryepiglottic fold hemangioma

目的探讨杓会厌襞血管瘤理想的手术方法。方法采用全麻下颈侧入路，对8例杓会厌襞血管瘤进行手术治疗。为扩大暴露，术中切除部分患侧甲状软骨板，直视下沿肿瘤包膜外分离后切除。切缘黏膜用可吸收线对位缝合，创面缺损较大者用梨状窝黏膜修复。结果8例患者病变均完整切除，无并发症。随访2～5年无复发。结论颈侧入路是切除杓会厌襞血管瘤理想的手术治疗方法。

Objective To explore an ideal approach for surgical excision of aryepiglottic fold hemangioma. Methods Eight patients with aryepiglottic fold hemangioma were treated surgically through lateral cervical approach under general anesthesia. The tumor was removed with additional exposure by partial resection of the lamina cartilaginis thyroidea in the affected side. The cutting edge of the tumor was fixed by apposition suture. In cases of large surgical defect, reconstruction was then conducted with mucosa from the piriform sinus. Results Complete resection was obtained in all of the eight patients without complications. Follow-up demonstra-ted no recurrence. Conclusion The lateral cervical approach seems to be the ideal way for the resection of aryepiglottic fold hemangioma.
