摘要
用Meta-analysis方法评价非放射性分枝杆菌培养系统检测结核分枝杆菌耐受利福平的准确性。通过检索PubMed、EMBASE、CBMWeb、CSJD、CJFD数据库和其他方式广泛收集文献。用QUADAS(Quality Assessment of Diagnostic AccuracyStudies)标准评价文献质量。用Meta-Disc软件计算合并灵敏度、合并特异性和SROC曲线等评价非放射性分枝杆菌培养系统的诊断正确性。最终纳入6篇文献。非放射性分枝杆菌培养系统在检测结核分枝杆菌耐受利福平时,均显示了较高的灵敏度和特异性。MB/BacT系统的合并灵敏度=100%,合并特异性=99%。BACTEC MGIT960检测系统的平均灵敏度=100%,平均特异性=91%,SROC曲线下面积为0.997。结论:MB/BacT和BACTEC MGIT960系统在检测结核分枝杆菌耐药(利福平)性方面显示了较高的灵敏度和特异性。推荐使用MB/BacT和BACTEC MGIT960系统代替BACTEC460检测系统作为结核分枝杆菌耐药(利福平)性的确诊试验。限于纳入研究的局限性,手工操作的分枝杆菌生长指示管(MGIT)系统的检验效能还有待进一步研究。
To evaluate the diagnostic value of Non-radioactive Mycobacterium Culture System detected the rifampicin resistance in mycobacterium tuberculosis,a fully recursive literature search was conducted in PUBMED, EMBASE, CBMWeb, CSJD and CJFD. Quality Assessment of Diagnostic Accuracy Studies (QUADAS) items were used to evaluate the quality of included studies. Meta-disc software was used to handle data from included studies. The SEN, SPE and SROC were used to assess the diagnostic value of Non-radioactive Mycobacterium Culture System. The six studies were included finally. Non-radioactive Mycobacterium Culture System showed the better SEN and SPE. The summarized SEN and SPE of MB/BacT System and BACTEC MGIT 960 System were 100%, 99%, 100%, and 91%respectively. The SROC of BACTEC MGIT 960 System was 0. 997. The results indicate that BACTEC 460 System can be replaced by MB/BacT and BACTEC MGIT 960 System to be as final diagnosis test of rifampicin resistance in Mycobacterium tuberculosis. The diagnostic values of MGIT System need to be studied further in future because of the limitation of included studies in small sample and quality.
