Clinical Value of Troponin I in Patients with Acute Miocardial Infarction

目的研究一种具有高度敏感性和特异性的血清标记物,能够早期对急性心肌梗死患者进行检测和诊断。方法收集400例诊断为急性心肌梗死患者的血液标本,分别对所采血液进行肌钙蛋白(IcTnI)和肌酸激酶同工酶MB(CK-MB)水平的定量检测,研究其阳性率、敏感性和特异性。结果阳性率:cTnI为93.4%,CK-MB为32.1%;敏感性:cTnI为100%,CK-MB为48.2%;特异性:cTnI为93.4%,CK-MB为32.1%。结论cTnI阳性结果对急性心肌梗死患者发生具有高度的预见性,cTnI的阴性结果显示急性心肌梗死患者发生机率很小。cTnI可作为心肌细胞受损时一种高度敏感性和特异性的血液标记物。cTnI可以早期预见急性心肌梗死(AMI)患者微血栓的形成,cTnI也可以作为一种排除不稳定型心绞痛患者发生心肌梗死的标记物。

Objective To study a highly sensitive and specific blood marker,it can earlier examine and diagnose a patient with AMI. Methods Gather 400 patients blood sample with AMI. To respectively take its blood into be measured the cTnI or CK-MB levels, studying its positive rate, sensitivity and specificity. Results cTnI positive rate： 93.4%, sensitivity： 100%, specificity： 93.4% ; CK-MB positive rate： 32.1%, sensitivity ：48.2%, specificity： 32.1%. Conclusion cTn! appeared to have a high predictive value that AMI occurred,its sensitivity is 100%. cTnI -negative value indicates very little risk of AMI occurrence; cTnI is the highly sensitive and specific blood marker as myocardial cell was injured. It can early foresee that the patient＇s tiny thrombus is formed. It is the marker which excludes patient＇s myocardial infarction.
