单纯倍频Nd∶YAG激光或与药物联合治疗皮肤血管瘤的临床疗效分析

Effects of Frequency-Doubled Nd∶YAG Laser Alone or Combined with Drugs Therapy on Superficial Hemangioma

目的探讨单纯倍频Nd∶YAG激光或与药物联合治疗皮肤血管瘤的效果及其影响因素。方法收集1999年至2006年期间采用单纯倍频Nd∶YAG激光或倍频Nd∶YAG激光联合药物治疗的皮肤血管瘤病例资料(共501例),并对治疗的结果进行回顾性分析。结果浅层皮肤血管瘤和联合方法的治疗效果分别优于深层皮肤血管瘤和单纯激光的治疗效果(P均<0.01)。同时显示,在一定范围内,病变的面积越大,治疗效果越差;激光的能量密度越高、治疗的次数越多、治疗效果越好(P均<0.05)。结论通过对激光参数的调整,加强血管瘤激光治疗适应证的选择以及与药物联合治疗,可提高血管瘤的治愈率。

Objective To observe the therapeutic efficacy of laser therapy alone or combined with drugs on the superficial hemangioma and their impact factors. Methods A total of 501 cases （552 lesions）that had been treated by two modalities have been collected and analyzed retrospectivly. Results The therapeutic effects of the combined method on the superficial hemangioma were more excellent than that of laser therapy alone and on the deep hemangioma（ OR = 3. 350, 0. 441, P 〈 0. 01 ）. The smaller the size Of hemangioma, the higher the energy of laser and the more the therapeutic times, the more the excellent therapeutic effects on hemangioma would be （OR = 1. 303, 0. 931, 0. 852, P 〈 0. 05）. Conclusions The cure rate of the superficial hemangioma can be improved by adjusting the parameters of the laser, selecting the correct cases and combining with other methods.
