腹膜后纤维化的临床及多层螺旋CT表现

Clinical Features and MSCT Findings of Patients with Retroperitoneal Fibrosis

目的探讨多层螺旋CT(MSCT)对腹膜后纤维化的诊断价值。方法分析经临床病理证实的11例腹膜后纤维化的临床及多层螺旋CT表现。结果11例患者中男性5例、女性6例,平均年龄52.7岁。特发性8例,继发性3例。多层螺旋CT平扫及经多平面重建,病变表现为腹膜后间隙或盆腔间隙的低密度、不均匀密度或等密度弥漫性浸润病变6例,不规则肿块样病变5例,增强检查显示为病变呈不同程度强化。6例伴有肾盂及输尿管扩张积水。结论多层螺旋CT及其多平面重建能很好显示腹膜后纤维化病变的各种表现,腹膜后间隙出现弥漫浸润性或不规则肿块样病变,并合并肾盂及输尿管扩张积水时应考虑到本病诊断。

Objective To study the diagnostic value of multi-slice spiral CT（MSCT） for patients with retroperitoneal fibrosis （RFP）. Methods Clinical features and MSCT findings of 11 cases of retroperitoneal fibrosis proved by clinks＇ and pathology were analyzed. Results There were 5 males and 6 females with an average age of 52.7 years;8 cases were idiopathic fibrosis and 3 cases were non-idiopathic fibrosis. Plain MSCT scanning and enhanced MSCT scanning were performed in all cases. Hypodensity, inhomodensity or isodensity diffuse lesions were showed in 6 cases and well circumscribed masses in 5 cases, and various enhancements were showed by enhanced scanning. 6 eases had the complications of hydronephrosis or hydroureterosis. Conclusion MSCT and its multiplanar reconstruction can show the features of retroperitoneal fibrosis. Retroperitoneal fibrosis should be considered if MSCT shows diffuse infiltrating lesion or well circumscribed masses in retroperitoneal space complicated with hydronephrosis or hydroureterosis.
