LISS钢板治疗股骨髁粉碎性骨折临床观察

摘要 目的探讨LISS钢板在股骨髁粉碎性骨折治疗中的应用价值。方法对我院对收治的37例C2、C3型(Muller股骨髁骨折分型)股骨髁粉碎性骨折患者,采用膝前正中切口显露复位关节面骨折端,然后采用LISS钢板固定;术后第3天开始进行膝关节功能恢复器锻炼。记录所有患者的骨折临床愈合时间及术后膝关节功能恢复情况。结果所有患者术后恢复顺利,均获随访,随访时间平均14.5个月(13—24个月),没有出现骨折不愈合或深部感染。骨折临床愈合时间平均为16.5周(15~20周)。术后1年参照膝关节外科临床评分标准,37例患者中:优16例,良17例,可3例,差1例,优良率为89.2%。结论对于股骨髁粉碎性骨折,利用膝前正中切口进行关节面的精确复位、采用单侧LISS钢板进行骨折固定,术后尽早进行膝关节功能锻炼能获得良好的疗效。 Objective To explore the treatment of comminuted fractures of femoral condyle with LISS. Methods From March 2005 to April 2007, 37 cases of type C2 and type C3 comminuted femoral condylar fractures (according to the Muller classification system ) were stabilized with LISS plate by anterior midline approach. The CPM was performed from the third clay postoperative. The average time for fracture healing and knee function were observed and documented. Results All cases were followed up from 13 months to 24 months with an average 14.5 months. No patient showed nonunion or deep infection. The average time for fracture healing were 16. 5 weeks (13 - 24 weeks). The knee function were assessed one year after operation according to the Knee Surgery Clinical Scoring System. The rates of excellent and good were 89.2%. Conclusion For surgical treatment of comminuted femoral condylar fractures, the stabilization with LISS plate by anterior midline approach and the early rehabilitation show good results.
