超声引导穿刺组织病理学检查对甲状腺结节诊断价值的探讨 被引量:18

摘要 目的:探讨超声引导穿刺组织病理学检查在甲状腺结节性病变诊断中的作用。方法:对2004-07-2005-01收治的41例患者进行甲状腺结节行超声引导下穿刺组织病理学检查的与术后大体常规组织病理诊断结果进行比较分析。结果:本组41例行超声引导穿刺组织病理学病例中有22例直接诊断完全正确,占53.7%;对甲状腺结节的良恶性判断方面上,超声引导穿刺组织病理学诊断的准确度达90.2%,灵敏度为71.4%,特异度为100%,阳性预测价值为1,阴性预测价值为0.90,Youden指数为0.714。无明显穿刺并发症。结论:超声引导穿刺组织病理学检查具有很高的术前诊断价值,值得临床应用。 OBJECTIVE:To evaluate the diagnostic usefulness of ultrasonography (US)-guided spring-activated biopsy in the thyroid gland. METHODS: Forty-one cases of thyroid nodule from Jul 2004 to Jan 2005 were studied by US-guided spring-activated biopsy. The results of routine pathological examination of gland species were set as golden standard to assess the sensitivity, specificity, et. of US-guided spring-activated biopsy in thyroid nodules. RESULTS: Twenty-two cases of the 41 cases (53.7%) were completely diagnosed accuracy.The Acc,Sen,Spe,PV +, PV-,Youden index of conventional pathology of the 41 spring-activated biopsies were 90.2%,71.4%,100%,1,0.90 and 0.714, respectively. No obvious complication by puncture was found. CONCLUSION: The US-guided spring-activated biopsy has high preoperative diagnostic value and is worthy of recommendation.
