摘要
目的评价弥散加权成像(DWI)参数图直观鉴别脑内常见环状强化病变良恶性的临床应用价值。方法回顾性研究包括脑脓肿14例和脑肿瘤16例(其中脑胶质瘤14例、转移瘤2例)共30例脑内环状强化病变的磁共振DWI参数图特征,所有病变均经手术病理证实。根据病变良恶性和DWI参数图中环状强化区内的信号高低进行分组,采用四格表x^2检验进行统计学分析。结果在DWI参数图上,14例脑脓肿中12例(86%)直观显示为高信号,2例(14%)为低信号;16例脑肿瘤中,15例(94%)直观显示为低信号,1例(6%)为高信号。统计学分析结果表明脑脓肿和脑肿瘤中环状强化DWI参数图直观显示的信号高低组间差异有统计学意义(P<0.05)。结论在临床工作中,磁共振DWI参数图在脑内环状强化病变良恶性鉴别诊断中具有直观、特征性的应用价值,脑脓肿的环状强化病变在DWI参数图上多直观表现为高信号,而脑胶质瘤或转移瘤的环状强化病变多显示为低信号。
Objective To evaluate the clinical application of diffusion weighted imaging (DWI) map in intuitively differentiating intracerebral opt-malig ring-enhanced lesions. Methods Magnetic resonance DWI parameter map of 30 patients showing ring-enhanced intracerebral lesions diagnosed by histopathologic findings were studied retrospectively, including 14 opt-lesions (abscesses) and 16 malig-lesions (14 gliomas and 2 metastases). According to the nature of lesions and high or low characteristic of DWI map signal intensities, the data were subgrouped and analyzed by tabular form X^2. Results In the DWI parameter map, 12 of 14 opt-lesions (abscesses) (86%) showed hyperintensity of region of ring-enhanced lesions, and 2 of 14 (14%) showed hypointensity. 15 of 16 malig-lesions (13 brain gliomas and 2 metastases, 94%) were detected as hypointensity, and 1 of 16 mali-lesions (brain glioma, 6%) was detected as hyperintensity. Significant difference in signal features of DWI parameter map existed between intracerebral opt and malign ring-enhanced lesions (P〈0. 05). Conclusions In the clinical practice, DWI parameter map shows intuitively characteristic clinical values in differentiating intracerebral opt-malig ring-enhanced lesion. DWI map shows hyperintensity in abscesses, and hypointensity in brain gliomas or metastases.
