性激素药物治疗134例青春期功能性子宫出血的疗效分析

The effectual analysis of hormonal drugs in the treatment of dysfunctional uterine bleeding in adolescent patients

目的探讨炔诺酮及避孕药治疗青春期功能性子宫出血(功血)的止血效果、适宜的起始剂量、用药方法。方法对本院2002年1月至2007年5月治疗的134例临床资料进行回顾性分析。结果本组134例中采用炔诺酮(选用妇康片)组70例,联合型口服避孕药(选用妈富隆,COC)组64例。快速控制出血率和快速止血率炔诺酮组与COC相比无显著差异;控制出血时间和完全止血时间炔诺酮起始剂量小于或等于15mg/d组和大于15mg/d组相比无显著差异;控制出血时间和完全止血时间COC2～3片/天组和每8小时1片组相比无显著差异。结论用COC治疗青春期功血在控制出血方面效果与炔诺酮效果类似,增加炔诺酮剂量并不增强其止血效果,COC给药方法不同不影响止血效果。

Objective To investigate the clinical efficacy of norethisterone or contraceptive in the treatment of acute bleeding episode of dysfunctional uterine bleeding（DUB） in adolescent patients and the optimal starting dosage of the drugs. Methods The clinical records of 134 girls with DUB and moderate or severe anemia from January 2002 to May 2007 were retrospectively analyzed. Results All 134 patients received hormonal therapy. Seventy patients were treated with norethisterone, sixty four patients with combined oral contraceptive（COC）. The rates of rapidly controlling bleeding and the rates of rapidly stopping bleeding in norethisterone group were similar to those in COC group, The different starting dosages （≤15mg/d and 〉 15mg/d in norethisterone group） showed no difference in the time intervals for controlling bleeding and completely stopping bleeding; The same starting dosages but different therapy（2-3 piece/d and 1 piece q8h in COC group） showed no difference in the time intervals for controlling bleeding and completely stopping bleeding. Conclusion The clinical efficacy of norethisterone on DUB in adolescent patients is similar to that of COC. And the efficacy of norethisterone at different dosages is similar. The efficacy of COC at same dosage but different therapy is similar.
