摘要 目的:探讨脑卒中后抑郁障碍(PSD)患者1H-MRS的变化特征及规律。方法:对35例脑卒中后抑郁症患者及26例健康对照组行MRI、1H-MRS扫描,兴趣区为双侧额叶背外侧白质及前部扣带回皮质。结果:①PSD组前额叶背外侧白质内Cho/Cr比值明显高于对照组(P<0.01);而NAA/Cr比值两组间差异无统计学意义(P>0.05)。②PSD组患者前额叶扣带回皮质内Cho/Cr比值明显高于对照组(P<0.01);而NAA/Cr比值两组间差异无统计学意义(P>0.05)。结论:1H-MRS能够揭示PSD时前额叶背外侧白质及扣带回皮质存在着细胞膜磷脂代谢的异常和细胞内信号转导异常。 Objective:To investigate the feature and rules of changes of post-stroke depression (PSD) in ^1H-MRS. Methods: Thirty-five patients with PSD and 26 healthy volunteers were scanned on a 1.5 T magnetic resonance scanner. Voxels were placed the bilaterally dorsolateral white matter and cingulate gray matter of the frontal lobe. Results:(1)The Cho/Cr ratios in dorsolateral white matter of the frontal lobe were significantly higher in PSD group patients than that in control group (P〈 0.01),but there were no statistical difference in the NAA/Cr ratio between the two groups (P〉0.05). (2)The Cho/Cr ratios in cingulate gray matter of the frontal lobe were significantly higher in PSD group patients than that in control group (P〈0.01), but there were no statistical difference in the NAA/Cr ratio between the two groups (P〉0.05). Conclusion:^1H-MRS can provide that altered membrane phospholipids metabolism and altered intra-cellular signal transduction may exist in dorsolateral white matter and cingulate gray matter of the frontal lobe.
