子宫内膜异位症宏观与微观辨治探讨

Discussion of the Relationship between Macroscopic and Microscopic Morphology in Endometriosis

子宫内膜异位症是指具有生长功能的子宫内膜组织出现在子宫腔被覆黏膜以外的身体其他部位而引起的疾病,主要临床表现为慢性盆腔疼痛、不孕、盆腔包块以及月经不调等。中医通过对疾病症状的综合分析,推究其内在的病理变化,以外揣内,由表达里,进行辨证论治,这种辨证方式称为宏观辨证。随着现代医学先进检查技术(如影像学检查、内镜检查、实验室检查、病理组织检查、基因检查等)的发展,可以从器官水平、细胞水平、亚细胞水平、分子水平、基因水平等较深层次上探讨疾病的本质,把宏观辨证与微观辨证有机地结合起来,建立有效与科学的临床诊断与疗效评价体系,为中西医结合诊断、治疗子宫内膜异位症搭起桥梁,在一定程度上补充和完善中医传统的宏观辨证内容。

Endometriosis （EMs） is the disease of endometria tissue, which can grow up, occuring out of cavitary uteri mucosa. The main symptoms of it include chronic pain in cavitas pelvis, barrenness, lump in cavitas pelvis and irregular menses. Doctors analyze the symptoms of the diseases and figure out its pathological change in order to determine treatment based on pathogenesis. This differentiation way is named by macroscopic syndrome differentiation. With the development of advanced modem check techniques （such as imageology, Endoscopy, laboratory examination, histopathological analysis, gene test and so on ）, microcosmic syndrome differentiation is thought highly more and more of by scholars. Microcosmic syndrome differentiation explains the essential qualities of the disease in the view of organic level, cellular level, subcellsular level, molecular level and genic level. It combines macroscopic syndrome differentiation with microcosmic syndrome differentiation, and sets up effective and rational clinical diagnostic system and curative effect evaluation system. It builds up a bridge for diagnosing and treating endometriosis with the therapy of combination of TCM with western medicine. And it also supplies and consummates the contents of traditional Chinese medicine on macroscopic syndrome differentiation.
