摘要
目的探讨肝脏悬吊法进行肝脏恶性肿瘤半肝切除的可行性与安全性。方法16例临床确诊的肝脏恶性肿瘤患者,解剖肝后间隙,通过肝后下腔静脉与肝脏之间的间隙置带并悬吊肝脏进行半肝切除。结果16例患者中15例应用悬吊法进行半肝切除,平均手术时间261 m in,平均出血量550 m l,术中术后均无并发症发生。术后随访有3例复发。结论应用悬吊法进行肝脏恶性肿瘤半肝切除是安全可行的,其关键是对肝后间隙的解剖学有深入的认识。
Objective To explore the feasibility and safety of hemihepatectomy with liver hanging maneuver for malignant tumor. Methods Hemihepatectomy was performed in 16 cases diagnosed as liver malignant tumor by hanging the liver with a tape through the tunnel between the retrohepatic inferior vena cava and the caudate lobe. Results Hemihepatectomy with liver hanging maneuver was performed in 15 of 16 cases with average operation time 261 rain, with average blood loss 550 ml and without intraoperative and postoperative complication. 3 recurrent case was confirmed with postoperative follow-up. Conclusions Hemihcpatectomy with liver hanging maneuver was safe and feasible for liver malignant tumor, in which the identification of retrohcpatic anatomy played an important role.
