原发性翼状胬肉的两种手术治疗的疗效比较

The comparison of two kinds of operation style on primary pterygium

目的比较单纯胬肉切除术基础上分别联合白体角膜缘干细胞移植术和应用丝裂霉素c治疗原发性翼状胬肉的临床疗效。方法将102例122只眼患者随机分为两组，在单纯胬肉切除术基础上分别采用角膜缘干细胞移植术（50例60只眼，简称干细胞移植组）和应用丝裂霉素C（52例62只眼，简称丝裂霉素组），术后随访1～2年。结果角膜上皮平均愈合时间：干细胞移植组为（4．0±0．8）d，丝裂霉素组为（7．3±1．7）d，两组差异有统计学意义（t=14．08，P〈0．05）。干细胞移植组2只眼复发，复发率为3．3％，丝裂霉素组14只眼复发，复发率为22．58％，两组差异有统计学意义（χ^2=9．91，P〈0．05）。结论单纯胬肉切除术，联合自体角膜缘干细胞移植术治疗原发性翼状胬肉角膜创面修复快，复发率低，是一种理想的手术青法佰，里椎广府胃．

Objective To compare the clinic effects of corneal limbal stem cell transplantion and simpie pterygium excision plus mitomycin C on primary pterygium.Methods One hundred and two patients（ 122 eyes ）were divided into two groups randomly.Corneal stem cell transplant group included 50 patients（60 eyes ）, while comnon ptergium excision plus mito mycin C included 52 patients （62 eyes ）.Postoperative follow-up time was 1-2 years.Results The average epithelial recovering time of corneal wound were 4.0± 0.8 days （stem cell group）and 7.3 ± 1.7 days （mitomycin C group）respectively.The differences of two groups were significant（ t =14.08, P 〈0.05 ）.Two eyes relapsed in the stem cell transplant group（recurrence rate was 3.3% ）, and 14 eyes relapsed in the mitomycin C group（recurrence rate was 22.58% ）, the differences was significant（χ^2=9. 91 ,P 〈0.05 ）.Conclusion The corneal limbal stem cell transplantion is an ideal surgical method on primary pterygium for the shorter time ofrecovering and lower rate of recurrence.
