摘要
目的掌握经翼点入路1颈内动脉外侧间隙的显微外科解剖。方法在15例(30侧)经颈内动脉灌注红色乳胶的成人尸体头颅上模拟翼点入路,测量第三间隙的数据,并探讨在不同角度下对基地动脉顶端分叉的暴露情况,进行显微解剖。结果经翼点入路第三间隙,可对颈内动脉床突上段、后交通动脉、脉络膜前动脉、基底动脉顶端等重要结构进行良好的显露。结论经翼点入路第三间隙在一定的角度下,可以清晰暴露基底动脉顶端,但需要保护好穿动脉。
Objective To study the microsurgical anatomy of exposing interspace lateral to internal carotid artery of the pterional approach. Methods Interspace lateral to internal carotid artery was examined through the operating spaces in 15 formalin-fixed adult eadavenc heads in which the arteries were perfused with red silicone and discussing the exposing of the basilar apex with different perspeetives by mieroanatomy. Results Interspace lateral to internal carotid artery of the pterional approaeh can provide exeelent exposure for the important structures such as the supraelinoid portion of the internal earotid artery, posterior communicating artery, posterior cerebral artery and the apex of the basilar artery. Conehlsion We can clearly expose the apex of the basilar artery through the interspace lateral to internal carotid artery of the pterional approach in a eertain angle, but the perforating branches of the carotid should be protected earefully.
