摘要
目的探讨小儿消化性溃疡急性穿孔的临床特点、诊断和治疗方法。方法回顾性总结和分析25例小儿消化性溃疡急性穿孔的临床资料。结果25例患儿临床症状和体征无特异性,72.0%(18/25)病例发病于春夏之交或复习应考期间,术前确诊10例(40.0%)、误诊15例(60.0%),全部病例均行单纯穿孔修补术而痊愈出院。结论小儿消化性溃疡急性穿孔的临床表现无特异性,早期诊断较为困难,X线、B超、腹腔穿刺等检查可提高诊断率;治疗以手术为首选,宜用单纯穿孔修补术。
Objective To investigate the clinical characteristics and diagnosis, treatment of acute perfora.tion of peptic ulcer （APPU） in children. Methods The clinical data of 25 cases with APPU were analysed retrospectively. Results The clinical symptoms and signs has no specific, 18 cases （72.0%） of 25 APPU occurred in period between spring and summer or before exam.The diagnosis was made preoperatively in I0 cases（40.0%）, and rnisdiagnosed in the other 15 Cases（60.0%）. All patients underwent perforation repairing operation. Conclusion The clinical manifestation of APPU in children has no specific clinical presentation. The early diagnosis is difficult. The simple perforation repair should be chosen at first.
