Expression and significance of Muc-1,Muc-2,Muc5AC,Muc-6 in hyperplastic polyp,globoid dysplasia,gastric signet ring cell carcinoma

摘要 目的研究癌前病变增生性息肉、球样异性增生和胃印戒细胞癌组织的粘蛋白表达规律和相关性及其临床病理意义。方法收集我院近5年的胃镜活检和肿瘤切除标本67例,其中增生性息肉20例,球样异性增生13例,胃印戒细胞癌34例,分别制作组织芯片。采用免疫组织化学(SP)方法检测各组中Muc-1、Muc-2、Muc5AC、Muc-6粘蛋白表达部位和强度。结果Muc-1、Muc-2在增生性息肉、球样异性增生和胃印戒细胞癌三者之间均存在相关性,随着病变发展,Muc-1表达率呈依次下调趋势。Muc-2表达率呈上调趋势。Muc5AC表达在增生性息肉与胃印戒细胞癌存在相关性,随着病变发展,Muc5AC表达率呈下调趋势。Muc-6与上述没有特殊相关性。结论Muc-1、Muc-2、Muc5AC有助于解释三者间存在增生-异常增生-癌的病理变化过程,进而便于对增生性息肉、球样异性增生癌前病变患者进行管理及定期随访。 Objective To elucidate the expression of Muc-1, Mue-2, Mue5AC, Mue-6 in the gastric signet ring cell carcinoma and precancerous lesions including hyperplastie polyp and globoid dysplasia and its clinicopathological significance. Methods Gastroscopie biopsy and surgery samples of patients with gastric signet ring cell carcinoma (n=34), globoid dysplasia (n=13) and hyperplastie polyp (n=20), treated in recent five years were retrieved. The tissue samples of each group were then used to make tissue mieroarrays (TMA). SP method of immunohistoehcmica] technique was used to detect expression Mue-1, Mue-2, Mue5AC, Mue-6 in TMA sections. Results There was a significant correlation between the three groups in the expres- sion of Mue-1, Mue-2.As precancerous lesions development to signet ring cell carcinoma, the expression of Mue-1 decreased, while Mue-2 was in the opposite way. There was a significant correlation between the hyperplastie polyp and gastric signet ring cell carcinoma in Mue5AC,with the down-regulated expression tendency. Conclusion Muc-1, Mue-2, Mue5AC may contribute to explain the pathological pathway of hyperplasia to dysplasia and finally to carcinoma. Furthermore, they may be convenient for follow-up programme performance for the patients with precancerous lesions including hyperplastie polyp and globoid dysplasia.
