摘要
目的探讨颅底肿瘤术后脑脊液中内皮素-1(ET-1)和一氧化氮(NO)的动态变化及其与术后脑血管痉挛(CVS)发生的关系。方法选择诊断明确的颅底肿瘤患者33例,术中术后留取脑脊液标本,测定脑脊液中ET-1和NO的含量,同时行颅超声多普勒(TCD)检查以判定CVS。结果颅底肿瘤术后发生CVS的患者第3天脑脊液中ET-1水平即有明显升高,第7天达到高峰,以后逐渐下降。CVS组ET-1水平明显高于无CVS组,症状性CVS组ET-1水平高于无症状性CVS组。颅底肿瘤术后发生CVS的患者第3天脑脊液中NO水平明显降低,术后第7~10天达到最低,以后逐渐升高。CVS组NO水平明显低于无CVS组,症状性CVS组低于无症状CVS组。结论颅底肿瘤患者术后脑脊液中ET-1水平的升高和NO水平的降低参与了术后CVS的发生,其含量与病情演变、CVS程度有关。
Objective To evaluate the dynamic changes of eotaxin - 1 ( ET - 1 ) , nitric oxide ( NO ) in cerebrospinal fluid ( CSF ) and their relationship with cerebral vasospasm ( CVS ) after skull base tumor resection. Methods 3 3 patients with skull base tumors receiving surgical treatment were chosen. Tile concentration of ET-1 and NO in CSF was measured during operation and at different time- points after operation. TCD was applied to determining whether CVS occurred in these patients before and after operation. Results After operation, ET-1 level in CSF of patients with CVS got markedly elevated on the 3^nl day, and reached a peak on the 7^th day, and then decreased gradually. The ET-1 level in CSF of the CVS group was significantly higher than that of the group without CVS. The ET-1 level in CSF of symptomatic CVS subgroup was higher than that of asymptomatic CVS subgroup. As for NO , its level in CSF of patients with CVS markedly decreased on the 3^nl day, and reached a nadir within 7 to 1 0 days after operation, and then increased gradually. The NO level in CSF of the CVS group was significantly lower than that of the group without CVS. The NO level in CSF of symptomatic CVS subgroup was lower than that of asymptomatic CVS subgroup. Conclusion The results suggest that the increase of ET-1 and the decrease of NO in CSF might be involved in the pathogenesis of the operation- associated CVS , and their levels might relate to the evolution of the patients ' pathogenetic condition and the degree of the CVS.
